The Ilizarov technique utilizes a circular external fixator in order to stabilize bony segments. The Ilizarvov apparatus (fixator) is comprised of a series of external rings attached to one another via threaded rods attached through adjustable nuts. Tensioned wires and half pins are transfixed to the bone and are also attached to the external rings stabilizing the entire bone.
Due to the circumferential construction and tensioned wires of the system, the Ilizarov external fixator provides superior mechanical strength and more structural support and stability than the traditional mono-lateral fixator system. As a matter of fact, it allows early weight-bearing in lower extremity applications which promotes healing. Some of the Ilizarov technique advantages other other treatments include:
In addition to support and strength, the Ilizarov apparatus is designed to permit rings to change position relative to one another. Because the rings are attached to bone segments via wires and half pins, moving the rings has the effect of moving bone segments.

The ability to precisely control the movement of bone segments is unique to the Ilizarov method and allows your foot and ankle surgeon to compress, distract and lengthen various portions of a bone throughout treatment. The Ilizarov technique is common in the treatment of complex problems such as complex ankle fractures, fracture nonunions, bone infections, bone lengthening, foot deformities, distraction osteogenesis, and more.
Ilizarov external fixation has also been proven to be effective to address problems associated with neurologic foot and ankle deformity with infection. It allowed eradication of infection and achievement of a stable plantigrade foot. (2)
In the United States, the Ilizarov technique is a highly specialized technique used mainly for deformity correction by experienced surgeons due to its complexity. Further development of the ring construct led to the Taylor Spatial Frame, an innovative ring fixator that uses six struts to connect the rings that have simplified the correction of complex deformities.
